On 01/06/2024 Officer William Forberg with the Shakopee Police Department ("Officer Forberg"), was dispatched to an assault report at St Francis Hospital in Shakopee, Scott County, Minnesota, Dispatch advised that a doctor and nurse was assaulted by a patient in the Behavior Health (BH) room. The suspect was identified Taleen Mohamed Zeidan Hassan [DOB REDACTED] ("Defendant"). The victims were identified as PY a Doctor and JM who assisted in the administration of the medication. Officer Forberg spoke to a witness identified as RSK who was a nurse on scene and witnessed what happened.
RSK walked Officer Forberg to see PY. After he walked back, he observed JM and RSK provide medication to Defendant. Defendant began thrashing around and yelling at both JM and RSK. He observed JM holding Defendant's arm, and RSK admitting the medicine. The two came back and advised that Defendant had dug her nails into JM and left marks, similar to those that she left on PY. Officer Forberg then interviewed JM about what happened. JM stated the following: - JM went into room BH1 with RSK to assist in providing medicine to Defendant. - Defendant was uncooperative. - JM grabbed Defendant's right arm to allow for RSK to safely administer the medication. - While holding the arm, Defendant began to scream vulgar and threatening language at JM. - JM specifically highlighted that Defendant yelled "I will kill you" at her. - Defendant then used her right hand and dug her nails into JM's left hand. - Defendant's actions left a mark on JM's hand in a form of a nail mark.
Officer Forberg also spoke with PY. Prior to speaking with him, Officer Forberg noticed redness and possible scratch marks on his right hand. PY stated the following: - Defendant was roaming within the BH secured area. - PY and other staff decided to place Defendant into her bed and into four-point restraints. - PY grabbed Defendant's right arm and assisted getting her into her bed. - While PY was holding down Defendant's arm, she was able to dig her nails into PY's right hand. - Defendant dug her nails into PY's hand, leaving nail and scratch marks. - Defendant was eventually restrained without causing further harm.
While Officer Forberg was still on scene, staff called another "code green", as Defendant was out of her restraints, screaming obscenities at staff and threatening harm. Officer Forberg observed as they were able to safely re-restrain her without further injuries. Officer Forberg then took a formal statement from RSK who witnessed the assault on PY. She stated the following: - Staff was attempting to restrain Defendant due to her behavior. - PY had taken control of Defendant's arm while they attempted to place her in restraints. - While PY was holding Defendant's arm, Defendant dug her nails into the top of PY's hand. - PY was initially unable to let go of Defendant's arm, as it was not yet restrained, and it was possible she could strike him if her let go.
