On April 26, 2026 at 12:19 A.M., Faribault Police Officer Mohamed Nur was dispatched to the Faribault Hospital for a report of an assault. Officer Mohamed Nur spoke with Adult Female Victim [NAME REDACTED] at the hospital. Adult Female Victim [NAME REDACTED], JUAN BERNARDO GUERRERO [DOB REDACTED], the defendant herein, had assaulted her about an hour prior to speaking with the officer. Adult Female Victim [NAME REDACTED] had run from the location of the assault. Adult Female Victim said the defendant drives a Mazda MPV. Adult Female Victim said the defendant does not own the vehicle and the license plate is a whiskey plate.
Adult Female Victim explained that she and the defendant had been at his residence in Farmington the previous day. Today they had driven down to Faribault. Prior to the assault, the two were in the area of Division Street W. And 2nd Avenue NW, longboarding. At some point, the defendant struck Adult Female Victim [NAME REDACTED]. This caused her to fall to the ground. Adult Female Victim said that she saw black and when she regained awareness, she saw the defendant running away from her. Officer Mohamed Nur observed multiple injuries to Adult Female Victim's face.
Adult Female Victim [NAME REDACTED] only struck her once in the face and the rest of the injuries were due to the fall. Adult Female Victim said a friend transported her to the hospital. Adult Female Victim [NAME REDACTED] was not the first time the defendant has assaulted her. Adult Female Victim said February 6, 2026 was the most recent incident in which the defendant assaulted her. Adult Female Victim [NAME REDACTED] on her birthday. Prior to leaving the hospital one of the doctor's told Officer Mohamed Nur Adult Female Victim had sustained fractures to her nose.
Officer Nelson shared information with Officer Mohamed Nur that at approximately 11:30 P.M. On April 25, 2026, he observed a vehicle parked along the west side curb in the 10 block of 2nd Avenue SW. Officer Nelson noted the driver's side front door was open. The officer did not notice anyone near the vehicle. The vehicle had MN LIC WT2747. Both officers returned to that area. Officers Mohmed Nur and Nelson observed fresh blood on concrete ramp on the south side of Division Street W. The blood trails went towards Division Street West's south curb.
Adult Female Victim was transported to HCMC for further evaluation. The reports from HCMC indicate Adult Female Victim [NAME REDACTED]. Adult Female Victim reported during the exam that her boyfriend hit her in the face while they were walking under the bridge.
