Your complainant is an officer with the Minnesota State Patrol. In that capacity, I have reviewed the police reports of Trooper Perbix and believe the following facts to be true. On November 6, 2025, at approximately 3:21 P.M., Trooper Perbix responded to a vehicle crash that was blocking westbound Highway 212, east of Great Plains Boulevard, in the City of Chanhassen, Carver County, Minnesota. Trooper Perbix was informed that a male had left their vehicle bearing Minnesota license plate number MUX314, and was running away from the crash.
Trooper Perbix located the male and instructed him to remain where he was. The male, later identified as JASON TREVORI CORK, [DOB REDACTED], the defendant herein, continued walking towards Trooper Perbix and began digging in his pockets. Trooper Perbix displayed his taser and instructed the defendant to get on the ground. The defendant got on the ground and showed Trooper Perbix that he was unarmed by pulling down his pants and exposing himself. The defendant was adamant that he was not the driver of the vehicle that had crashed.
When asked why he had the vehicle’s keys, the defendant stated that he “took them out of the car.” While speaking with the defendant, Trooper Perbix detected the odor of an alcoholic beverage emanating from his person. The defendant admitted to consuming one shot. The defendant agreed to perform some field sobriety tests, which he was unable to perform satisfactorily. The defendant agreed to provide a breath sample for the PBT, which registered a fail reading, and the defendant was placed under arrest for driving while under the influence of alcohol.
The defendant was read the Minnesota Breath Test Advisory and stated that he understood it and would take a breath test, the results of which indicated an alcohol concentration of.13 at 4:46 P.M. A witness, BL, reported that he observed the defendant’s vehicle drifting from lane to lane, and stated that the vehicle’s tires crossed over the fog and lane lines. BL observed that the vehicle was unable to maintain its speed, going plus or minus 20 miles-per-hour at times. BL also observed the crash, and stated that he could not see a passenger, only a driver in the defendant’s vehicle.
BL stated that the driver was male. Another witness, CS, stated that he observed a single male occupant in the defendant’s vehicle. CS stated that the male seemed “out of it,” and asked him to take him away from the crash. When CS told the male that he would not, the male left the scene. A check of the defendant’s driving record reveals prior impaired driving conviction(s), prior (alcohol-related) loss of license, or an ordinance in conformity therewith on October 3, 2024. This check also revealed that the defendant’s driver’s license was suspended, and as of November 6, 2025, had not been reinstated.
