(7/29/1999), the above-named defendant, and the allegations contained therein. Based upon that information, your complainant believes the following to be true and correct. On December 21, 2025, at 3:21 PM, Ofc. Brandau of the Rochester Police Department took a report from Victim- an adult female fully identified in police reports, involving past-action domestic abuse. Victim reported a significant history of abuse at the hands of ex-boyfriend, Leodis Henry Jr. (7/29/1999), Defendant herein. Victim described the relationship as involving repeated instances of physical abuse, emotional manipulation, and financial control.
Victim reported that she had not previously filed police reports for these incidents but had documented some of them through photographs, videos, personal notes, and a hospital visit at Saint Mary's Hospital (SMH). The incidents reported took place in both Minnesota and Florida. Victim specifically reported an incident on March 30, 2025, taking place in a residence in SE Rochester, Olmsted County, Minnesota. On that date Victim attempted to leave the residence to visit family. Defendant restrained Victim in their bedroom by grabbing her wrists and her body, pushing her on the floor, and straddling her.
Victim stated that Defendant raised his right hand and struck Victim in her in the face in the area of her upper jaw with his elbow. Victim reported that her jaw was fractured and three teeth were dislocated and fractured. Victim was unable to close her mouth properly and experienced severe pain. Victim showed Ofc. Brandau videos of photographs of these injuries which depicted blood on her face, apparent loose teeth, and blood in her teeth. Defendant brought Victim to the hospital, but Victim was not initially truthful about the cause of her injuries due to fear of Defendant.
After Defendant had left, she was truthful about the injuries with medical staff. Det. Powell was assigned to follow up on the case. Det. Powell obtained medical records from the incident reported by Victim. The records indicated Victim presented with "apparent dental fractures has a result of an assault. Patient states that she was struck in the mouth by an elbow. She remembers the event. Was not knocked out. Denies any other injuries." PLEASE TAKE NOTICE: YOU MUST APPEAR FOR EVERY COURT HEARING REGARDING THIS CASE. FAILURE TO APPEAR FOR COURT IS A CRIMINAL OFFENSE AND MAY RESULT IN ADDITIONAL CRIMINAL CHARGES BEING IMPOSED AND PUNISHED AS PROVIDED IN MINNESOTA STATUTES SECTION 609.49.
